A.) The time it takes to move your voice and/or data services to another location varies from carrier to carrier so you should verify with your particular provider.  Most carriers will request at least a 30 day notification if you would like to move your services to a new address.

It can take longer too. It depends on a lot of things incuding the new location. What services are available there already? Does your provider already have fiber/service there or do they need to wait for another provider to provide part of the connection. (local loop/last mile in case of T1 or larger connections)
I've also seen cases where it get's done much faster too. Be sure to work with your provider and let them know as soon as possible that you are looking to move. If you have it narrowed down to a couple sites, give them the address and let them check for service availability and ETA.
